Case presentation A 42-year-old woman, at 16 weeks of gestation, presented during her routine antenatal visit with a two-week history of sudden-onset bilateral visual blurring. Her visual acuity had declined to the point where she could see only shadows and needed help with mobility and daily tasks. Despite these symptoms and profound fatigue, she had not sought earlier medical attention, assuming the issue was not serious. Previous hospitalization Four weeks prior, she had been hospitalized for nausea and vomiting complicated by a urinary tract infection, but discharged herself against medical advice.

Since then, she had been having ongoing non-projectile vomiting, occurring two to three times a day, primarily involving food particles and gastric contents, without any blood. She could tolerate only sweetened beverages and fruit juices, vomiting even small amounts of solid food. This…
