A 25-year old male, undergraduate student, with no systemic conditions, presented with a primary complaint of “short teeth and excessive exposure of gums when smiling”. Image description: Primary complaint of gummy smile and short crowns. Clinical exam demonstrated short and square-shaped crowns of the maxillary incisors and a wide band of keratinized gingiva. Mean probing depth in this sextant was 2 mm.
On visual exam , there was no evidence of vertical maxillary excess. The crown size was not compatible with his ectomorph body type (tall and thin). When performing the transulcular periodontal probing under anesthesia (bone sounding), it was possible to determine that the alveolar crest was at the same level or close (<1.5mm) to CEJ level. *This case is from Docplexus editorial team for educative purpose only Source: Trends in Clinical Periodontology
