Case Presentation-  On 18th June 2025, a 74-year-old male, who is my regular patient and a chronic smoker, was seen in a corporate hospital at 3 AM with chest pain. ECG showed RBBB pattern. ECHO was reported as NAD. Troponin I was 4.7 units in that laboratory. He developed chest pain again in the morning after coming home. A repeat Troponin I was done and reported as 1696 units in that laboratory.

Based on this, he was advised CAG by the cardiologist. Investigations The patient underwent CAG, which showed an 80% calcification block in LAD. Management He underwent angioplasty using cutting technology. Discussion The discussion here is related to the exponential increase in Troponin I within a short span of time, and the value was extraordinary.
