I heard about this incident from my friend where this patient went to his cardiologist for a two-week follow-up. The patient informed his doctor that he was having trouble with the application of the transdermal patch that was prescribed to him to relieve his chest pain. He explained that the nurse had instructed him to put a new patch every six hours and that he was running out of places to put the patch on. After asking patient to undress, doctor found the unthinkable.

Patient had not removed any of his older patches and had about twenty-five to thirty patches all over his body. Later, doctor made sure to add instruction while prescribing the transdermal patch that was to remove the old patch before applying the new one. I am sure, we will experience similar incidences during our medical practice. What do you think?
