A patient from Bihar 84yr/m had fever for 10 days which responded to ceftraixone. After 5-6 day he again developed fever with mouth ulcer. This time low TLC with 90% lymphocytes. For agraulocytosi, ceftazidime started; after not responding changed to meropenem with amikacin with ofloxacin. Inj fluconazole started.

Fever not responding for last 15 days. Tlc gradually decreasing to 2.8×10^3. Malaria negative( one dose provided), kalaazar rk39 negative. What to do?
