Case presentation A 35-year-old previously healthy man reported a two-month history of progressively worsening lower back pain. The pain was dull, non-radiating, and accompanied by reduced spinal mobility, significantly interfering with his daily routine, and graded as 2β3 on the Graded Chronic Pain Scale. Over time, he experienced urinary hesitancy, a feeling of incomplete bladder emptying, and mild lower abdominal discomfort that occasionally presented as sharp, cramping pain. He denied any hematuria, fever, weight loss, or previous urological disorders.
Physical examination Localized tenderness over the lumbosacral spine was noted, with no motor or sensory neurological deficits Digital rectal examination The prostate was found to be enlarged, firm, and irregular on palpation.
