An 86-year-old woman presented with 3-week-old itchy rashes on her trunk and arms. She had been treated for breast cancer previously and her medical history showed glaucoma and cataracts. She had no history of tuberculosis, sarcoidosis, atopic diathesis, or granuloma annulare. She had not taken any new medication over the last few weeks. No fever or night-chills were reported. Physical examination revealed the following: Pink urticarial like papules Plaques on trunk and arms Punch biopsy from her left shoulder showed a sarcoptic mite which was present in association with granulomatous dermatitis.

A burrow for Sarcoptes mite was found within the stratum corneum while granulomatous dermatitis was present within the dermis. The granulomas were surrounded by lymphocytes and scattered eosinophils. She tested negative for infectious organisms. What could be the probable diagnosis? *This case…
