History A 32-year-old female presented to the hospital eight days after normally delivering her second child. She suffered from gradually worsening headache that started a night before. She also suffered from pleuritic bilateral rib pain, which was worse on the right side, and was associated with shortness of breath and leg edema.
Observation No signs of fever, vomiting or stroke-like symptoms Examination Pulse – 58 bpm Blood pressure – 177/95 mmHg Respiratory rate – 22 Temperature – 97.8◦F Diminished breath sounds on the right 1+ bilateral pitting leg edema without redness Neurological exam – normal Laboratory workup normal except D-dimer >20, aspartate aminotransferase (AST) 100 U/L and alanine aminotransferase (ALT) 150 U/L Electrocardiography shows sinus bradycardia.
