Thoracic epidural analgesia was administered via catheter to two children who were to undergo surgery subsequently. Post-operatively, both the children presented with unilateral facial flushing and hyperhidrosis with distinct demarcation along the facial midline (Figure 1). The contralateral side presented with pallor and anhidrosis. The physical examination revealed no abnormality; there was no ptosis, and the pupils were symmetrical in both the patients.
