An 88-year-old male presented with abdominal pain and fever with chills. He had a medical history of atrial fibrillation, hypertension, obesity, hyperlipidemia, chronic kidney disease, left ventricular hypertrophy, aortic and mitral valve disease and benign prostatic hyperplasia. A dusky, large cecal tumor was observed which was covered with fibrinous exudate. There was no overt focal perforation.
