An 82-year-old presented with an enlarged left testis for three months. There was no tenderness, heaviness, or scrotal pain. He reported no fever or abnormal urethral discharge as well. Physical examination: Prominent enlargement of testis Firm and hard left testis No inguinal lymphadenopathy Sonography revealed diffuse heterogeneous density in the left testis. LDH, B-HCG, and AFP were normal. An abdominal to pelvic CT scan image has been provided.

No distal metastasis or lymph node enlargement was noted, and the patient denied any family history of testicular cancer. No symptoms or signs of gynecomastia were observed. Histological images of tumor lesions (X4 and X40) have been provided. Immunohistochemical stain of the tumor was positive for both Vimentin and inhibin-alpha, and Bcl-2.
