A 22-year-old male presented with recurrent episodes of hemoptysis since 4 weeks. He reported that the initial symptom of cough which was followed by bouts of coughing with around 2-4 tbsp. blood. He was initially thought to be a case of epistaxis secondary to allergic rhinitis and treated accordingly. There were no other associated symptoms such as fever, weight loss or gain, chest pain, skin complaints, dyspnea, or rash.

Past History: The patient complained of chronic postnasal drip, nasal congestion, and occasional pruritus but thought to be allergic. He has a past history of eosinophilic colitis, OCD, aneurysmal bone cyst status post-removal, and allergic rhinitis. He was recently diagnosed with hypogonadism and was on injectable testosterone supplements. No history of smoking, alcohol, drug abuse, inhalant exposure or airway trauma.
