Case presentation A 63-year-old female presented to her doctor with chronic and intermittent abdominal pain from which she had been suffering for the last eight years. Her pain, which she described as sporadic, was also associated with bloating. She, however, did not experience any nausea or emesis.

Physical examination: Non-specific tenderness (poorly localized) to abdominal pain No signs of constipation, diarrhea, blood in her rectum, or weight loss Laboratory examination: Hemoglobin - 8.2 g/dL Haematocrit - 28.3% Iron level - 20 μg/dL TIBC - 514 μg/dL Iron saturation - 4% Platelets - 452,000/μL Fecal occult blood testing – Negative Because of her iron deficiency anemia, she was being orally and parenterally administered iron supplementation. She had also been using NSAIDs for a prolonged time. Prior colonoscopy showed internal hemorrhoids and mild diverticulosis but no polyps.…
