A 49-year-old small stature and dysmorphic featured woman presented with transient loss of consciousness. The patient has Turner’s Syndrome with no previous history of diabetes. She was admitted to the inpatient department and underwent a controlled 72 hours fast. After 64 hours, she developed hypoglycemia, at 30 mg/dl, with emotional outbursts.
The biochemical evaluation showed unsuppressed insulin levels (1.2 mIU/L), markedly elevated proinsulin (30 pmol/L), and beta-hydroxybutyrate (BOHB) levels of 6500 umol/L.
