A 38-year-old man reported to the Ophthalmologist due to reduced vision in his left eye after he had received intraoral local anesthesia with articaine hydrochloride and epinephrine (1: 100,000) in his left upper jaw. He had been given a local infiltration anesthesia during treatment of dental caries in his upper left maxillary canine (cuspid). A self-aspirating injection syringe (25 G, 0.6 × 25 mm) had been used to perform aspiration before the injection. Shortly after the injection, he started experiencing a headache, loss of visual acuity and gradual worsening of vision.

This was accompanied by occasional flashing stars in his left visual field. He denied nausea or phonophobia and no periapical infection was noted in the anesthetized tooth.
