A 41-year-old woman presented with acute pain, nausea, and vomiting for the last twelve hours. Her medical history included a total laparoscopic hysterectomy which she had undergone two years ago. Mild distension and tenderness were observed over the right lower quadrant, and the pain was noted in the right vaginal fornix upon examination. The vaginal examination also revealed the presence of a painful adnexal mass in the rectovaginal pouch of Douglas. In addition, a cystic lesion was observed in the right ovary along with moderate ascites in transvaginal ultrasonography.
