Synopsis In a major population of HIV-infected patients, tuberculosis has been found to be the most common opportunistic infection. Although early intervention of antiretroviral therapy (ART) in HIV–infected patients with tuberculosis is responsible for reduced mortality, it increases the risk of paradoxical tuberculosis-associated immune reconstitution inflammatory syndrome (IRIS). This immunopathologic reaction occurs shortly after the initiation of ART in patients receiving antituberculosis therapy.
