A 53-year-old woman presented with a 10-day history of progressive, severe right neck and shoulder/scapular pain. The pain was worse with movements and radiated to her right arm and fingers. No significant past medical history or history of trauma, infections, and smoking was reported. She took NSAIDs and trigger point injections and also tried a physiotherapy session, however, that did not yield a significant relief.

Investigations: X-ray (Rt. Shoulder):- Right superior mediastinal mass. CT (Contrast):- A 7.2 cm well demarcated heterogeneously enhancing mass with areas of fat attenuation and some prominent internal vessels within the right superior, posterior mediastinum.
