Case Presentation A 56-year-old male was hospitalized due to an increase in the swelling of the mass at the root of his right neck (Cover image a). A big pulsating mass on the right neck was discovered during physical examination, and there were no other notable clinical symptoms. Medical history The patient was an active smoker.

No history of chest pain or trauma. Radiographic findings  Transthoracic echocardiography revealed an ejection fraction of 35% (Cover image b) Angio-CT scan indicated a 65 x 60 x 170 mm thrombosed aneurysm arising from the innominate trunk, with no involvement of the right carotid and subclavian arteries (Cover image c) Electrocardiogram revealed a prior inferior wall myocardial infarction with negative T waves.
