Highlights It is suggested that the topical use of tranexamic acid (TXA) may provide a high drug concentration on the wound surface with negligible systemic concentrations. However, a novel approach of moistening wound surfaces with TXA before closure has been shown to reduce the drain volumes by 39% in a study of bilateral reduction mammoplasty. Such moistening exposes the wound surface to TXA for a considerably shorter time than topical boluses.

This study demonstrates that moistening the wound with TXA 25 mg/ml before closure reduces postoperative bleeding within the first 24 hours of the surgery in patients undergoing mastectomy.
