Case presentation: A 55-year-old woman presented to the emergency room with complaints of stomach pain, chills, and fever . She had an unexplored 1-month history of rectal bleeding. She had been treated for rheumatoid arthritis with rituximab and was administered piperacillin-tazobactam in combination with gentamicin to treat sepsis.
