A 33-year-old male presented with a complaint of shortness of breath. The patient denied any fever or trauma. He was initially hypoxic and required 3 liters of oxygen. The patient’s medical history was positive for tobacco abuse, one pack per day over the past 15 years, marijuana abuse off and on during the past 10 years, and he worked as a handyman. Initial laboratory tests, including complete blood count, complete metabolic profile, cardiac enzymes, brain natriuretic peptide, and electrocardiogram, were within normal limits.
