A 66-year-old man was observed to have a mass measuring about 0.9 cm on the gallbladder wall. The mass did not show any increase in size for two years raising the possibility of it being a gallstone that is embedded in the gallbladder wall. A cholecystocolonic fistula involving the transverse colon was observed during cholecystectomy. The resected specimen had an opened gallbladder (4.5 × 3.0 × 3.0 cm) which had a mural thickening fragment of fibromuscular tissue, 2.5 cm long colon with a 1.5 cm mixed cholesterol gallstone. Chronic cholecystitis was observed in the sections of the gallbladder.
