Emphysematous pyelonephritis (EPN) is a rare life-threatening disease diagnosed radiologically with a mortality rate of up to 50%. It is a necrotizing infection of the renal parenchyma and the surrounding areas which results in the accumulation of gas in the renal parenchyma, collecting system or perinephric tissue. Emphysematous pyelonephritis (EPN), the rare infection of the kidney, generally occurs in diabetic mellitus (DM) patients or patients with uncontrolled diabetes.
In some occasional cases, it occurs in patients without DM along with the corresponding obstruction of the renoureteral unit or immunocompromised state. It is more common in females and is usually caused by the most common organisms, the glucose-fermenting bacteria Escherichia coli , Klebsiella pneumonia and Proteus mirabilis .
