A 36-day-old female infant developed respiratory distress and fussiness. She had been a term infant with a negative New York State newborn screen. Physical examination: Heart rate: 260 beats/minute Temperature: 32.4 o C Prolonged capillary refill Faint distal pulses She was given adenosine twice a day for supraventricular tachycardia, intubated for worsening respiratory distress, and placed on continuous positive airway pressure via nasal prongs and in FiO 2  of 1.0. Echocardiogram showed decreased biventricular systolic function along with left to right shunting via patent foramen ovale.
